Permanent Jewelry Consent & Acknowledgment
I voluntarily choose to receive permanent jewelry from Sparkles by Krystal.
By signing below, I acknowledge that:
- I have disclosed any metal allergies, skin sensitivities, implanted medical devices (including pacemakers), or other conditions that may affect this service.
- I understand the jewelry is welded while being worn and may produce a small spark.
- I understand permanent jewelry can be removed at any time by cutting the jump ring or chain.
- I understand Sparkles by Krystal is not responsible for accidental breakage, snagging, stretching, loss, or damage occurring after I leave my appointment.
- I certify that I am at least 18 years old or have the consent of my parent or legal guardian.
Warranty & Policies
- One complimentary reweld within 30 days of purchase (proof of purchase required). Original chain must be returned and be in reusable condition.
- Lost chains, replacement chains, and damage beyond rewelding are not covered.
- All sales are final.
Third-Party Re-weld Policy
I understand that my permanent jewelry was originally installed by another jeweler or business. By requesting a repair from Sparkles by Krystal, I acknowledge that Sparkles by Krystal is responsible only for the new weld completed during this repair.
The repaired weld is covered by a complimentary 30-day reweld warranty from today’s date. This warranty does not cover the original chain, links, connectors, charms, or any unrepaired portion of the jewelry. Future repairs or replacement may incur additional charges.
